The admission process is one aspect of connected care that often proves challenging for emergency medicine and hospitalist teams. In order to facilitate admissions and optimize patient care, ApolloMD promotes the use of bridge orders when appropriate.
The collaborative development of bridge orders is one phase of the ApolloMD EM – HM synergy program which enhances teamwork between the two departments. Through the program, the teams meet to discuss barriers to admission and agree upon a process, delineating each team’s responsibilities and the point upon which the patient transitions from emergency to inpatient care.
The implementation of effective bridge orders enhances communication, throughput, and clinical quality, positively impacting both the emergency physician, hospitalist, and patient.
